Job summary

Emerald Healthcare & Rehab Cozad is seeking a skilled Physical Therapist to join our rehab team on a PRN basis in Cozad, NE. Qualifications

• DPT or Master’s degree in Physical Therapy required.
• Current state licensure in good standing.
• Experience in skilled nursing or long-term care preferred.

Responsibilities

• Assess patients' physical conditions, mobility, and functional abilities.
• Develop and implement individualized therapy programs to improve function and reduce pain.
• Administer therapeutic exercises, manual therapy, and modalities as prescribed.
• Monitor patient progress and adjust treatment plans as needed.
• Educate patients and families on home exercise programs and safety.
• Collaborate with physicians, nurses and other team members to coordinate care.
• Document evaluations, treatment plans, progress notes and discharges in the chart.
