Job summary

Breezy Hills Rehabilitation and Care Center in Lakeland, FL is seeking a compassionate Speech-Language Pathologist (SLP) to join our rehabilitation team on a PRN basis, providing speech, language, cognitive, and swallowing therapy to residents in a skilled nursing setting.

The qualified candidate holds a graduate degree in Speech-Language Pathology, Florida licensure (or eligibility), and ASHA CCC-SLP eligibility; SNF experience is preferred.

Qualifications

• Graduate degree from an accredited Speech-Language Pathology program.
• Florida Speech-Language Pathology license (current or eligible).
• ASHA CCC-SLP eligibility (or CCC-SLP holder).
• Experience in SNF, skilled nursing, rehabilitation, or healthcare preferred.
• Strong communication, clinical, organizational, and interpersonal skills.

Responsibilities

• Evaluate residents and develop individualized speech, language, cognitive, and swallowing treatment plans.
• Provide therapy to improve communication, cognition, swallowing, and functional independence.
• Treat residents with speech, language, voice, and dysphagia/swallowing disorders.
• Develop and implement therapeutic programs based on each resident's needs, abilities, and goals.
• Recommend and train residents in the use of adaptive and augmentative communication devices when appropriate.
• Monitor resident progress and modify treatment plans as needed.
• Collaborate with nursing, therapy, dietary, and other interdisciplinary team members.
• Participate in care plan meetings, staffing, and resident care conferences.
• Educate residents, families, and facility staff on communication and swallowing strategies.
• Supervise supportive personnel in accordance with professional and state regulations.
• Maintain accurate clinical documentation and comply with facility, state, and professional standards.
