Eden Senior Care, LLC is seeking a PRN Occupational Therapist to join Edenbrook Fond du Lac, a skilled nursing facility within the Eden Senior Care Network, to provide rehabilitation services.

The role requires evaluating residents, supervising COTAs, and implementing treatment plans to maximize ADL/IADL independence while guiding families and caregivers. Wisconsin licensure required; prior SNF rehab preferred.

Formación

• Master's or Doctorate degree in Occupational Therapy (OT) from an accredited program.
• Active Wisconsin OT license in good standing.
• Experience in skilled nursing, home health, or rehabilitation settings preferred.

Responsabilidades

• To provide quality Occupational therapy services and initiates referrals when appropriate, in accordance with established standards of practice, therapy department procedures, and productivity standards.
• To evaluate residents to obtain data necessary for treatment planning and conduct specialized evaluations as indicated.
• To supervise and direct Certified Occupational Therapy Assistants (COTAs) in the implementation of the treatment plan.
• Apply interventions to maximize safety and performance in activities of daily living (ADL) and instrumental activities of daily living (IADL)
• Recommend adaptive equipment and train in its use
• Guide and educate family members and caregivers
• Observe, note, and report on progress of long and short term treatment goals

Conocimientos

Active Wisconsin OT license

SNF/Home health rehab experience
