Job summary

Enhance Rehabilitation in Illinois is seeking qualified Physical Therapists to join our SNF team. You will evaluate and treat patients, develop individualized plans of care and set functional goals while collaborating with facility staff to ensure high-quality care.

You will supervise Physical Therapist Assistants, re-evaluate progress, adjust plans, and maintain timely documentation of evaluations, re-evaluations, discharges, and daily notes, contributing to patient outcomes and family

Qualifications

• Qualified Physical Therapist with SNF experience preferred.
• Ability to evaluate and develop plans of care.
• Strong communication with patients, families and staff.
