Job summary

Enhance Rehabilitation in Caseyville, IL is seeking qualified Physical Therapists to join our growing SNF team. You will evaluate and treat patients following an individualized plan of care, providing high quality care and education to patients and families.

You will collaborate with facility staff, supervise Physical Therapist Assistants, re-evaluate progress, revise plans of care, and complete timely documentation including evaluations, re-evaluations, discharges and daily progress notes.

Qualifications

• Evaluating new patients and developing plans of care with measurable goals.
• Providing high-quality patient care and education on injuries and outcomes.
• Supervising Physical Therapist Assistants and documenting progress notes.

Responsibilities
