Job summary

Enhance Rehabilitation is seeking qualified Physical Therapists to join our dynamic team in SNF settings, evaluating and treating patients according to individualized plans of care.

You will provide excellent patient care, collaborate with facility staff, supervise assistants, document evaluations, re-evaluations, and progress notes, and educate patients on their injury and prognosis.

Responsibilities

• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
• Collaborate with facility staff to ensure excellent care for you patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Supervising Physical Therapist Assistants
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
