Job summary

CommuniCare Rehab at Annandale Healthcare Center is seeking a Physical Therapist (PT) for PRN coverage. The clinician will evaluate residents, develop and implement individualized treatment plans, and collaborate with an in-house rehab team across multiple centers.

Qualifications

• Bachelor’s, Master’s or Doctorate Degree in Physical Therapy from an Accredited Program.
• Hold a valid or pending State Physical Therapy License.

Responsibilities

• Evaluate, review, and screen patient’s medical charts and evaluations; develop a treatment plan.
• Implement treatment plan through direct care and educate the plan of care to the rehab team; supervise PTAs as needed.
• Lead interdisciplinary meetings to relay clinical findings and make rehab recommendations; train staff and families when appropriate.
• Work across multiple centers to serve a wide range of patient needs.

Qualifications

• A Bachelor's, Master’s, or Doctorate Degree in Physical Therapy from an Accredited Program.
• Hold a valid or pending State Physical Therapy License

Job Responsibilities

• Evaluate, Review, and Screen patient’s/resident’s medical charts, evaluations under physician order and develop a treatment plan in accordance with patient’s needs.
• Implement treatment plan through direct treatment, as well as education of the plan of care to all team members, including providing necessary supervision for PTAs.
