Evolve Therapy Services seeks a Physical Therapist for an in-house Skilled Nursing Facility in Johnstown, PA on a PRN basis. You will deliver physical therapy services to residents and ensure care aligns with CMS expectations and company procedures.

Qualifications

• BS in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure.

Responsibilities

• Provide physical therapy services to residents in accordance with established standards of practice and department procedures.
• Adhere to documentation and billing procedures for physical therapy services.
• Demonstrate sound judgment in evaluation, planning, and follow-up of resident therapy programs.
• Conduct resident screenings to determine need for intervention/treatment.
• Develop treatment plans with short and long-term goals and methods to achieve them.
• Communicate evaluation findings and recommendations to residents, families, and the interdisciplinary team.
