Job summary

Powdersville Post Acute is a 60-bed skilled nursing and rehabilitation facility in Easley, SC. We are seeking a dedicated Physical Therapist to join our rehabilitation team on a PRN basis, evaluating and treating residents and creating personalized plans to maximize mobility and independence.

The role emphasizes collaboration with an interdisciplinary team, continuous documentation, and ongoing education for residents and families.

Qualifications

• Bachelor's degree in Physical Therapy required.
• Master's or Doctorate degree in Physical Therapy preferred.
• Experience in skilled nursing, rehabilitation, or long-term care preferred.
• Experience with Casamba and PointClickCare (PCC) preferred.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide physical therapy services in accordance with physician orders and established goals.
• Document evaluations, treatment sessions, progress notes, and discharge summaries accurately and timely.
• Monitor resident progress and modify treatment plans as appropriate.
• Communicate with physicians, nursing staff, residents, families, and other healthcare professionals regarding treatment plans and outcomes.
• Participate in care conferences, rehabilitation meetings, and discharge planning.
• Educate residents, families, and caregivers on therapy goals and home exercise programs.
• Conduct in-service training and education for facility staff as needed.
• Supervise Physical Therapist Assistants (PTAs), aides, and students in accordance with state regulations.
• Ensure compliance with state practice acts, facility policies, and regulatory requirements.
