Job summary

Solutions Rehab and Therapy in partnership with Southgate Nursing and Rehab is seeking a compassionate Physical Therapist (PT) to join our rehabilitation team. In this PRN role, you’ll help residents rebuild strength, restore mobility, and reclaim independence through personalized care and evidence-based therapy.

The ideal candidate has graduated from an accredited Physical Therapy program, holds an Illinois PT license, current BLS, and strong clinical, communication, and organizational skills.

Qualifications

• Graduation from an accredited Physical Therapy program.
• Active Physical Therapist license in Illinois.
• Current BLS certification.
• Excellent clinical, communication, and organizational skills.

Responsibilities

• Perform comprehensive physical therapy evaluations and establish individualized treatment plans.
• Deliver high-quality patient care using proven therapeutic techniques.
• Set and adjust short- and long-term functional goals.
• Maintain timely and accurate documentation of services provided.
• Collaborate with the interdisciplinary team to ensure coordinated, patient-centered care.
