Job summary

Enhance Rehabilitation seeks qualified Physical Therapists to join our team in the SNF setting. You will evaluate and treat patients following an individualized plan of care, collaborating with facility staff to ensure the highest quality of care.

You will supervise Physical Therapist Assistants, re-evaluate progress, and maintain timely documentation. Excellent patient service and communication with patients and families are essential.

Qualifications

• Evaluating new patients, developing plans of care and setting measurable, functional goals.
• Providing high-quality patient care in SNF settings.
• Completing timely and accurate documentation including evaluations, re-evaluations, discharges and daily progress notes.

Responsibilities

• Providing excellent care for our patients.
• Providing excellent customer service to our patients and family members.
• Collaborate with facility staff to ensure excellent care for your patients.
• Evaluating new patients, developing plans of care and setting measurable, functional goals.
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes.
