Job summary
Concept Rehab, Inc. is seeking a PRN Physical Therapist to join our Hart, Michigan skilled nursing team.
In this flexible PRN role, you will evaluate residents, develop individualized treatment plans, deliver evidence-based PT interventions, and collaborate with physicians and nursing staff to support mobility and quality of life. Candidates must hold a Michigan Physical Therapist license and have graduated from an accredited PT program; new graduates are encouraged to apply.
Qualifications
- Current Physical Therapist license in the State of Michigan.
- Graduate of an accredited Physical Therapy program.
- Excellent communication and interpersonal skills.
- Skilled nursing or geriatric experience preferred, but new graduates are encouraged to apply.
- Commitment to providing compassionate, patient-centered care.
Responsibilities
- Evaluate residents and develop individualized treatment plans
- Deliver evidence-based physical therapy interventions
- Collaborate with physicians, nursing staff, families, and the rehab team
- Complete timely and accurate documentation
- Educate patients and caregivers on treatment goals and home exercise programs
- Help residents maximize mobility, independence, and quality of life
