Qualifications

• Current state licensure or eligibility for licensure as a Physical Therapist.
• Active CPR Certification, Fingerprint card, 2-step Tuberculin test.
• New Graduates are encouraged to apply.
• Strong clinical reasoning and decision-making skills for diverse conditions.
• Excellent interpersonal and communication skills; collaborates with residents, families and team.

Responsibilities

• Evaluate residents' physical abilities, functional status and rehab needs.
• Develop personalized treatment plans based on assessments and goals.
• Implement interventions to maximize independence and safe mobility.
• Provide one-on-one PT sessions using varied techniques.
• Monitor progress and adjust plans to optimize outcomes.
• Collaborate with physicians, nurses, OTs and SLPs for coordinated care.
• Educate residents and families on exercises and home programs.
• Document evaluations, progress notes and discharge summaries per policy.
• Participate in care conferences and in-service training.
• Contribute to a positive, professional work environment.
• Perform all other duties as assigned.
