Job summary

Ridge Valley Center for Nursing & Rehabilitation is seeking a PRN Physical Therapist to deliver resident-centered rehabilitation. You will evaluate residents, develop treatment plans, and provide skilled therapy to improve mobility and independence within a collaborative team.

This flexible PRN role requires strong clinical judgment, excellent communication, and the ability to work with residents, families, and multidisciplinary staff to support quality outcomes in a skilled nursing setting.

Qualifications

• Graduate and licensed PT with strong clinical judgment.
• Experience in skilled nursing or rehab settings preferred.
• Able to work collaboratively with an interdisciplinary team.

Responsibilities

• Perform comprehensive evaluations based on physician referrals and resident needs.
• Develop individualized treatment plans with measurable, functional goals.
• Provide skilled one-on-one physical therapy interventions to improve mobility, strength, balance, endurance, and independence.
• Monitor resident progress and modify treatment plans as needed to achieve optimal outcomes.
• Participate in discharge planning and provide education to residents and families regarding safety and continued care.
• Complete timely and accurate documentation in accordance with standards and policies.
• Maintain treatment records and billing documentation.
• Stay current with Medicare guidelines and reimbursement requirements.
• Collaborate with physicians, nursing staff, OT, SLP, and other team members to coordinate care.
• Participate in care conferences, quality improvement initiatives, and team meetings.
• Communicate effectively with residents, families, and healthcare professionals to support positive outcomes.
• Supervise PTAs, rehab technicians, and students; provide guidance on restorative techniques and safe mobility.
• Assist with staff education, in-services, and professional development activities.
• Maintain a clean, organized, safe therapy environment and report equipment concerns.
• Provide coverage at nearby facilities as needed.
