Job summary

The Center for Living and Rehab in Vermont is seeking a PRN Physical Therapist to join our in-house rehab team. You will evaluate residents, develop PT plans, deliver skilled services to improve mobility and function, and document progress in our EMR.

We value resident-centered care in a collaborative, team-oriented setting and encourage ongoing education. Vermont license required; SNF or long-term care experience preferred. This position offers flexible PRN hours to fit your schedule.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Active Vermont Physical Therapist license (required).
• Experience in SNF or long-term care preferred.
• Strong communication, organizational, and clinical skills.
• Experience with EMR systems preferred.

Responsibilities

• Evaluate residents and develop individualized physical therapy treatment plans
• Deliver skilled therapy services to help improve mobility, strength, and overall function
• Monitor progress and adjust treatment plans based on resident response
• Educate residents, caregivers, and staff on therapy goals and safety techniques
• Document evaluations, treatment sessions, and outcomes accurately and timely using electronic medical records
• Collaborate with interdisciplinary team members to support resident care plans
