Job summary

ALC Home Health is seeking a PRN Physical Therapist to perform home-based PT services across designated geographic areas. The role supports flexible scheduling and direct collaboration with the Home Health Director for streamlined visit coordination.

Pay includes standard visits at $75.00 and evaluations at $85.00, with a $2,500 sign-on bonus. The therapist will assess, treat, and educate patients per physician plans of care, and participate in discharge planning and care coordination.

Qualifications

• Requires degree in physical therapy from an accredited program.
• Illinois license to practice physical therapy is required.
• Two years of physical therapy experience preferred; home health experience is a plus.
• Must have good communication and organizational skills.
• CPR certification is required.

Responsibilities

• Provide PT services per physician's plan of care and assess functional status.
• Direct PT treatment, educate patients and families, and determine priorities.
• Lead PT program and train others as needed.
• Refer patients to other home health services when appropriate.
• Prepare clinical progress summaries and participate in discharge planning.
• Provide PT consultation to families and staff and conduct in-service education.
