Job summary

ALC Home Health seeks a PRN Physical Therapist to deliver PT services on the road, following physician plans of care. The role offers per-visit compensation for standard visits and evaluations and includes a $2,500 sign-on bonus.

Responsibilities include patient assessment, treatment planning, instruction for patients and families, and collaboration with the Home Health Director to coordinate care. A valid PT license and CPR certification are required.

Qualifications

• Degree in physical therapy from an accredited program.
• Licensed to practice as a PT in the state.
• At least two years of PT experience; home health preferred.
• Strong verbal and written communication, organized.
• CPR certification current and maintained.
• Driver with insured vehicle; independent liability/workers comp coverage.
• Willingness to travel and flexible schedule.

Responsibilities

• Provide PT services according to physician’s plan of care; assess status and mobility.
• Evaluate range of motion, strength, gait; adjust therapy as needed.
• Direct therapy treatments and instruct patients/families on use of devices.
• Identify needs for additional home health services and refer accordingly.
• Prepare progress summaries and discharge planning documents.
• Offer PT consultation to families and provide in-service education.
• Supervise PT assistants and review care with home health aides every two weeks.
• Participate in peer consultations and maintain compliant documentation.
