Job summary

ALC Home Health is seeking a PRN Physical Therapist to deliver PT services in patients' homes under physician plans of care. You will assess, treat, and educate patients, and coordinate with families to optimize outcomes.

Flexibility to manage your own schedule and geographic area is offered, with administrative support and EMR-enabled visit tracking. Prior home health experience and active state licensure are preferred.

Qualifications

• Requires degree in physical therapy from an accredited program.
• Licensed to practice as a physical therapist in the state.
• Two years of relevant clinical/home health experience preferred.

Responsibilities

• Provide physical therapy services per physician-approved plan of care.
• Assess functional status, mobility, strength, ROM, and home environment.
• Direct therapy treatment and educate patients/families on therapy goals.
• Initiate therapy program and train staff and caregivers as needed.
• Participate in discharge planning and documentation of progress.
