ALC Health Care Services is seeking a PRN Physical Therapist to deliver professional PT services across home health visits in Illinois. The role pays per visit: $75 for standard visits and $85 for evaluations, with a $2,500 sign-on bonus.

You will work under a physician's plan of care and coordinate with a Home Health Director to schedule and track visits. Responsibilities include patient assessment, treatment planning, caregiver instruction, progress reporting, and discharge planning.

Qualifications

• Degree from a physical therapy program approved by an accredited organization.
• Licensed to practice as a physical therapist in the applicable state.
• Two years of PT experience; home health is preferred.
• Strong verbal and written communication and organization skills.
• CPR certification current.
• Valid driver's license with insured vehicle.
• Independent liability/workers' comp coverage/cert.

Responsabilités

• Provide PT services per physician's plan of care and assess patient status.
• Evaluate mobility, strength, ROM, and rehabilitation potential.
• Direct therapy treatment and instruct patient/family in use of devices.
• Determine priority PT needs and report changes to physician.
• Initiate PT program and educate other staff and family members as needed.
• Possibly train patient in prosthetic device use.
• Identify other home health service needs and refer as necessary.
• Prepare and submit progress summaries and goals attainment notes.
• Participate in discharge planning and case conferences.
• Provide PT consultation to families/caregivers when indicated.
• Supervise PT aides per policy and state regulations.

Physical Therapy degree (BPT/MSPT/DPT)
