Job summary

ALC Health Care Services is seeking a PRN Physical Therapist to deliver home health PT services across an assigned geographic area in line with the physician’s plan of care. Flexibility and administrative support are highlighted, with emphasis on patient outcomes and timely documentation.

Pay rates include Standard visits at $75.00 and Evaluations at $85.00, with on-the-road service responsibilities and a focus on efficient scheduling and communication with the Home Health Director.

Qualifications

• Bachelor’s or Master’s degree in Physical Therapy from an accredited program.
• Licensed to practice as a Physical Therapist in the state.
• Two years of PT experience; home health preferred.
• Strong verbal and written communication and organizational skills.
• CPR certification current.
• Must have a valid driver’s license and be insured.

Responsibilities

• Provides physical therapy services per physician’s plan of care; assesses functional status and contributes to the care plan.
• Evaluates strength, mobility, ROM; determines rehab potential.
• Directs PT treatment and educates patients and families on devices and exercises.
• Identifies needs for other home health services and makes referrals as needed.
• Prepares and submits clinical and progress summaries; participates in discharge planning.
• Provides PT consultations to families and caregivers and participates in peer reviews.
• Supervises PT assistants according to policy and state regulations.
• Instructs and supervises home health aides every two weeks when PT is sole skilled service.
