Job summary

The Visiting Nurse Association of Ohio Home Health is seeking a Physical Therapist to join their PRN team. This role offers the flexibility to set your own availability and provides competitive pay rates per completed visit. Must have an active PT license in Ohio and a minimum of one year of clinical experience. The role includes the provision of therapy services within patients' homes, and documentation must be completed in accordance with QA standards. Mileage reimbursement is also offered.

Qualifications

• Must hold an active PT license in Ohio and current BLS certification.
• Minimum of 1 year of clinical physical therapy experience required.
• Must have a reliable vehicle and valid driver's license.

Responsibilities

• Provide therapy services in patients' homes as per schedule.
• Complete required documentation in EMR within 24 hours.
• Ensure compliance with QA standards.
