Job summary

Aveanna Healthcare is hiring a Visiting Physical Therapist for our VNA Certified division to evaluate, coordinate, and deliver quality physical therapy in patients' homes. The role supports diverse populations across ages with chronic conditions and rehab needs.

You will provide therapies per physician orders, document progress, coordinate with care teams, and contribute to discharge planning. We offer flexible schedules, benefits, weekly pay, and mobility tools to support clinicians.

Qualifications

• Licensed PT in the applicable state.
• Current CPR certification (with hands-on) or able to obtain one.
• TB test current or able to obtain one.
• Must have a valid drivers license and reliable transportation.

Responsibilities

• Deliver physical therapy services per physician orders.
• Follow policies, procedures, licensure standards and state laws.
• Provide written evaluation defining needs, goals and outcomes with the referring physician.
• Coordinate services and discharge planning with the health care team, family and nurses.
• Maintain effective communication among staff and family.
• Serve as a resource for clinical staff and supervise PTA/HHA as applicable.
• Complete OASIS assessments where allowed by state practice.
