Job summary

The Care Team Hospice is seeking a compassionate Physical Therapist to support hospice patients in Akron, OH, and surrounding communities on a PRN, paid-per-visit basis.

This flexible role emphasizes comfort, safety and quality of life, with one-on-one visits to patients at home or in facilities. Candidates should be licensed in Ohio, have PT training, and be able to work independently while coordinating with an interdisciplinary hospice team.

Qualifications

• Active and unrestricted Physical Therapist license in Ohio.
• Graduate of an accredited Physical Therapy program.
• Previous physical therapy experience required; hospice/home health or geriatric experience preferred.
• Strong clinical assessment, communication, and patient education skills.
• Ability to work independently and provide care in patients' homes and facilities.
• Valid driver's license, reliable transportation and current automobile insurance required.

Responsibilities

• Complete individualized physical therapy evaluations focused on mobility, strength, balance, safety, functional abilities, and patient comfort.
• Develop and provide appropriate therapy interventions aligned with each patient's hospice plan of care and personal goals.
• Recommend assistive devices, safe transfer techniques, positioning strategies, and environmental modifications to improve comfort and safety.
• Educate patients, families, and caregivers on mobility, fall prevention, energy conservation, positioning, and safe daily activities.
• Collaborate with the interdisciplinary hospice team and complete accurate, timely clinical documentation.
