Job summary

Wellstar Health System is seeking a part-time Physical Therapist to assess, plan and participate in rehabilitative programs that improve mobility, relieve pain, and increase strength. The role entails collaboration with physicians, patient/family education, and discharge planning across inpatient settings.

Responsibilities include developing care plans, documenting treatments, and coordinating with staff for continuity of rehabilitation services.

Qualifications

• Masters in Physical Therapy is required.
• Doctorate in Physical Therapy is preferred.
• Minimum 1 year of Rehab, LTACH, Acute Care, SNF experience.
• BLS certification from ARC or AHA is required.

Responsibilities

• Establishes and evaluates treatment plans with the physician.
• Implements physical therapy treatment plan and progresses patient toward goals.
• Documents evaluation, plan of care, and each treatment session.
• Provides patient/family education, discharge instructions and referrals as appropriate.
• Acts as a referral source for staff and physicians on rehabilitation services.
