Job summary

Wellstar Health System, Inc. is seeking a Physical Therapist to assess, plan, and participate in rehabilitative programs aimed at improving mobility, reducing pain, increasing strength, and preventing deformity.

The role requires compliance with regulatory standards and involves work across the full continuum of care. Responsibilities include implementing the treatment plan, educating patients and families, documenting sessions, and coordinating discharge and referrals.

Qualifications

• Effective communication skills, customer service focused, team oriented, and basic computer skills.

Responsibilities

• Implements the physical therapy treatment plan.
• Selects appropriate treatment activities to progress patient towards goals.
• Revises goals/plan of care with patient/family input
• Provides discharge instructions, follow-up and referral to community resources as appropriate
• Acts as a referral source for staff, families, physicians on services and equipment related to rehabilitation services.
• Documents the evaluation and plan of care according to departmental guidelines
