Job summary

Wellstar Health System in Georgia is seeking a licensed Physical Therapist to assess, plan, and participate in rehabilitative programs that improve mobility, relieve pain, and increase strength. Services span the full continuum of care and emphasize quality, cost-conscious, and patient-centered delivery.

Responsibilities include implementing treatment plans, educating patients and families, coordinating with physicians, and ensuring compliance with regulatory standards.

Qualifications

• Active Physical Therapy license or eligibility in Georgia.
• Excellent communication and interpersonal skills.
• Patient-centered approach with good teamwork.

Responsibilities

• Implements the physical therapy treatment plan.
• Selects appropriate treatment activities to progress patient towards goals.
• Revises goals/plan of care with patient/family input.
• Provides ongoing patient/family education and training.
• Provides discharge instructions, follow-up and referrals as appropriate.
• Acts as a referral source for staff, families, physicians on rehabilitation services.
