Job summary

Collierville Nursing and Rehab is seeking a licensed Physical Therapist to provide therapy services and lead treatment planning for residents. You will work within a collaborative rehab team and maintain high standards of patient care.

Responsibilities include evaluation, treatment planning, supervision of PTAs and rehab staff, and ensuring compliance with regulations. A valid PT license and active engagement in continuing education are required.

Responsibilities

• Provide physical therapy services including screening, evaluation, and treatment.
• Comply with documentation and billing guidelines within the medical record.
• Establish and maintain patient-centered interactions with patients and staff.
• Refer patients to PTAs and provide supervision.
• Directly supervise PTAs and Rehab Techs per state acts and policies.
• Participate in utilization review, case management, and team activities.
• Develop and enhance PT programs and services.
• Ensure compliance with safety, infection control, and patient rights policies.
• Maintain licensure and stay informed on professional developments.
• Maintain confidentiality of patient information.
• Conduct duties ethically and professionally.
• Perform other duties as assigned.
