Collierville Nursing and Rehab in Tennessee is seeking a licensed physical therapist to deliver comprehensive PT services within our in-house program. You will screen, evaluate, and treat patients, develop treatment plans, discharge patients, and collaborate closely with nursing and rehab teams to support meaningful functional recovery.

Responsabilités

• Provide physical therapy services, including screening, evaluation, and treatment per referral.
• Develop and adjust initial and ongoing treatment plans of care.
• Discharge planning and supervision of assistants and rehab techs.
• Maintain professional conduct and adhere to regulatory requirements.
• Supervise PTAs and Rehab Techs in accordance with state guidelines.
• Participate in care conferences and interdisciplinary team activities.
• Ensure accurate documentation and billing within the medical record.
• Maintain patient rights and environmental safety compliance.
• Pursue ongoing education to enhance clinical expertise.
• Uphold patient confidentiality and ethical professional conduct.
