Job summary

Countryside Center for Rehabilitation and Nursing is seeking a licensed Physical Therapist to provide high-quality therapy services to residents. You will assess, treat, and develop patient-centered plans of care, while maintaining professional standards and compliance with state and federal regulations.

The role requires collaboration with nursing and rehab teams, supervision of PTAs and rehab techs, and ongoing professional development to enhance practice and patient outcomes.

• Collaborate with interdisciplinary team to optimize patient outcomes.
• Discharge planning and case management responsibilities.
