Qualifications

• Assist patients under the supervision of a Physical Therapist using an individualized plan of care.
• Deliver quality treatment aimed at achieving excellent functional outcomes.
• Record patient conditions and responses in charts after each visit.
• Communicate with Physi cal Therapist, Director/ Rehab, and facility staff.
• Operate and apply knowledge of standard physical therapy equipment.

Responsibilities

• Assist treating patients by following an individualized plan of care developed by Physical Therapists.
• Develop and implement physical therapy treatment programs based on each patient's needs.
• Assist patients with all phases of physical therapy treatment and techniques.
• Discuss treatment with patients and document progress after each session.
• Maintain continuous communication with the PT and rehab/facility team.
• Manage standard physical therapy equipment safely and effectively.
