Qualifications

• Clinical supervision by a Licensed Physical Therapist is involved.

Responsibilities

• Utilizes evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment to patients per regulatory and clinical practice requirements under supervision of a Physical Therapist.
• Delivers skilled therapy services to patients needing rehabilitation services.
• ASSESSES and documents patient progress and communicates with supervising Physical Therapist to adjust treatments.
• Provides counseling to patients and families regarding physical disorders and treatment plans.
• Contributes to discharge planning and documentation per regulatory requirements.

Some duties for this position include:

• Utilizes evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment intervention to patients with physical functioning disorders per regulatory and clinical practice requirements under the supervision and direction of an Physical Therapist.
• Provides skilled therapy services to patients who need rehabilitation services.
• Assesses and document patient’s progress and communicates with supervising Physical Therapist to adjust treatments accordingly.
• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to physical disorders.
• Provides input to supervisor for discharge summary of services provided per regulatory and clinical requirements.
