Job summary

Continuum Rehab Group in Texas is seeking a Licensed Physical Therapy Assistant to join our clinical team in Skilled Nursing facilities. You will apply evidence-based practice to deliver rehabilitative, medically necessary therapy under the supervision of a Physical Therapist.

The role welcomes therapists at all experience levels and emphasizes consistency of hours and patient-focused care to improve functional outcomes.

Qualifications

• Licensed PTA in Texas.
• Evidence-based practice knowledge.
• Ability to document patient progress.
• Communication with supervising PT.

Responsibilities

• Utilizes evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment interventions to patients under supervision of a Physical Therapist.
• Provides skilled therapy services to patients needing rehabilitation services.
• Assesses and documents patient progress and communicates with supervising PT to adjust treatments.
• Provides counseling to patients, families, and other providers related to physical disorders.
• Contributes to discharge summaries per regulatory and clinical requirements.

Experience/Qualifications:

Licensed Physical Therapy Assistant

This position is open to therapists at all experience levels.

Some duties for this position include:

• Utilizes evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment intervention to patients with physical functioning disorders per regulatory and clinical practice requirements under the supervision and direction of an Physical Therapist.
• Provides skilled therapy services to patients who need rehabilitation services.
• Assesses and document patient’s progress and communicates with supervising Physical Therapist to adjust treatments accordingly.
• Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to physical disorders.
• Provides input to supervisor for discharge summary of services provided per regulatory and clinical requirements.
