Job summary

Pine Acres Center for Nursing & Rehabilitation in North Carolina is seeking a licensed Physical Therapist (PT) for a PRN role. You will evaluate residents, develop individualized treatment plans, and deliver evidence-based therapy to promote functional recovery and independence.

The role offers flexible PRN scheduling, opportunities to work with an interdisciplinary team, and potential to cover nearby affiliated facilities as needed. North Carolina licensure is required or eligibility.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current North Carolina Physical Therapist license (or eligibility).
• Experience in skilled nursing, rehabilitation, or long-term care is preferred but not required.
• Strong clinical judgment, communication, and interpersonal skills.
• Ability to safely assist residents with transfers and mobility, including lifting up to 50 pounds when necessary.
• Commitment to providing compassionate, resident-focused care.

Responsibilities

• Perform comprehensive evaluations and develop individualized treatment plans based on physician referrals and resident needs.
• Provide skilled physical therapy services to improve strength, balance, mobility, endurance, and overall functional independence.
• Reassess resident progress and modify treatment plans to maximize outcomes.
• Supervise PTAs, rehabilitation technicians, and therapy students as appropriate.
• Complete accurate and timely documentation in accordance with facility policies, Medicare guidelines, and regulatory requirements.
• Collaborate with physicians, nurses, occupational therapists, speech therapists, and other members of the interdisciplinary care team.
• Educate residents, families, and caregivers on therapeutic exercises, mobility techniques, fall prevention, and home exercise programs.
• Participate in discharge planning, care conferences, and quality improvement initiatives.
• Provide coverage at nearby affiliated facilities as needed to support resident care.
