Job summary

Qualifications

Responsibilities

Position Summary

We are seeking a licensed Physical Therapist (PT) to provide skilled rehabilitation services to residents recovering from illness, injury, or surgery. In this PRN role, you'll evaluate residents, develop individualized treatment plans, and deliver evidence-based therapy that promotes functional recovery and improves quality of life.

This position offers flexible PRN scheduling while allowing you to work alongside an experienced interdisciplinary team dedicated to outstanding resident outcomes.

Key Responsibilities

• Perform comprehensive evaluations and develop individualized treatment plans based on physician referrals and resident needs.
• Provide skilled physical therapy services to improve strength, balance, mobility, endurance, and overall functional independence.
• Reassess resident progress and modify treatment plans to maximize outcomes.
• Supervise Physical Therapist Assistants (PTAs), rehabilitation technicians, and therapy students as appropriate.
• Complete accurate and timely documentation in accordance with facility policies, Medicare guidelines, and regulatory requirements.
• Collaborate with physicians, nurses, occupational therapists, speech therapists, and other members of the interdisciplinary care team.
• Educate residents, families, and caregivers on therapeutic exercises, mobility techniques, fall prevention, and home exercise programs.
• Participate in discharge planning, care conferences, and quality improvement initiatives.
• Provide coverage at nearby affiliated facilities as needed to support resident care.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current North Carolina Physical Therapist license (or eligibility for licensure).
• Experience in skilled nursing, rehabilitation, or long-term care is preferred but not required.
• Strong clinical judgment, communication, and interpersonal skills.
• Ability to safely assist residents with transfers and mobility, including lifting up to 50 pounds when necessary.
• Commitment to providing compassionate, resident-focused care.
