Job summary

Dahlia Gardens Center for Nursing & Rehabilitation in North Carolina is seeking a licensed Physical Therapist (PT) for a PRN position to evaluate residents, develop individualized treatment plans, and deliver evidence-based therapy in a skilled nursing setting.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current North Carolina Physical Therapist license or eligibility.
• Experience in skilled nursing, rehabilitation, or long-term care preferred.
• Strong clinical reasoning, communication, and interpersonal skills.
• Ability to assist residents with transfers and mobility up to 50 pounds.

Responsibilities

• Perform resident evaluations and develop individualized treatment plans.
• Provide PT services to improve strength, balance, mobility, and independence.
• Monitor progress and modify treatment plans as needed.
• Supervise PTAs, rehab technicians, and students as needed.
• Document accurately and timely per policy and regulations.
• Collaborate with physicians and interdisciplinary team members.
• Educate residents and families on exercises and home programs.
• Participate in discharge planning and quality improvement initiatives.
• Travel to nearby affiliated facilities to support resident care.
