Job summary

Aventura Health Group's Aventura at West Park in Cincinnati, OH is seeking a PRN Occupational Therapist (OT) to join our in-house rehabilitation team. You will evaluate residents, develop personalized treatment plans, and deliver skilled OT services to promote independence.

This role emphasizes collaboration with physicians, nursing staff, and families, with flexible PRN hours and opportunities to pick up additional shifts.

Qualifications

• Current Ohio Occupational Therapist license in good standing.
• Graduate of an accredited Occupational Therapy program.
• Experience in skilled nursing or long-term care is preferred but not required.
• Strong communication, organization, and clinical skills.
• Experience with Net Health and PointClickCare (PCC) is preferred.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide skilled occupational therapy services to improve functional independence.
• Collaborate with physicians, nursing staff, therapy team members, residents, and families.
• Maintain timely and accurate documentation in Net Health.
• Utilize PointClickCare (PCC) as part of the interdisciplinary care process.
• Ensure compliance with all state, federal, and company regulations.
