Job summary

Aventura Health Group is seeking a PRN Physical Therapist to join the in-house rehabilitation team at Shiloh Springs in Trotwood, OH. You will evaluate residents in skilled nursing and long-term care settings, developing plans of care and delivering mobility- and function-focused therapy.

Join a supportive team and contribute to resident education, accurate documentation, and seamless discharge planning while enjoying flexible PRN scheduling and a collaborative work environment.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Ohio Physical Therapist license in good standing.
• SNF/LTC experience preferred.
• Experience with Net Health and PointClickCare (PCC) preferred.
• Strong communication and teamwork skills.
• Dependable, compassionate, and resident-focused.

Responsibilities

• Evaluate residents and develop individualized physical therapy plans of care.
• Provide skilled PT services focused on mobility, strength, balance, safety, and functional independence.
• Collaborate with PTAs, nursing, physicians, and the interdisciplinary care team.
• Provide resident and family education as appropriate.
• Complete timely and accurate documentation.
• Support safe discharge planning and continuity of care.
