Job summary

Aventura at Shiloh Springs – Trotwood, OH is seeking a PRN Physical Therapist (PT) to join the in-house rehabilitation team. This role focuses on skilled nursing and long-term care residents with flexible PRN scheduling and competitive rates.

You will evaluate residents, develop plans of care, and collaborate with an interdisciplinary team while using Net Health and PCC for documentation. Ohio license required; SNF/LTC experience preferred.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Ohio Physical Therapist license in good standing.
• SNF/LTC experience preferred.
• Experience with Net Health and PCC preferred.
• Strong communication and teamwork skills.
• Dependable, compassionate, and resident-focused.

Responsibilities

• Evaluate residents and develop individualized physical therapy plans of care.
• Provide skilled PT services focused on mobility, strength, balance, safety, and functional independence.
• Collaborate with PTAs, nursing, physicians, and the interdisciplinary care team.
• Provide resident and family education as appropriate.
• Complete timely and accurate documentation.
• Support safe discharge planning and continuity of care.
