Job summary

Paulding Care Center is seeking a PRN Physical Therapist to join our nursing team. The role involves providing comprehensive PT evaluations and implementing treatment plans in accordance with physician orders, with emphasis on patient outcomes and family education.

Qualified candidates will hold a Georgia license in Physical Therapy and CPR certification is preferred. This on-site position offers opportunities to contribute to high-quality rehabilitation care in a supportive environment.

Qualifications

• Graduate of an accredited university with a B.S., Doctorate, MA or MS in Physical Therapy recognized by the APTA.
• Holds current license and/or registration in the state of Georgia.
• CPR certified preferred.

Responsibilities

• Provide a comprehensive physical therapy evaluation based on MD’s orders.
• Document findings on the standardized evaluation form, accurately and adhering to guidelines.
• Provide a comprehensive treatment plan including long and short term goals, frequency, duration, treatment modalities, family education and documentation.
• Ensure MD orders are obtained for evaluations, treatments, re-certifications and discharges.
• Provide comprehensive treatment to patients utilizing modalities and modify patient treatment as indicated while adhering to precautions.
