Job summary

National Church Residences is seeking a Physical Therapist for Home Health PRN coverage in Southern Ohio, including Ross and Pike counties. You will evaluate and treat seniors in their homes, develop personalized plans of care, and collaborate with an interdisciplinary team to promote independence.

Qualifications

• Degree in Physical Therapy (DPT, MS, or BS).
• 2–4 years of experience preferred.
• Valid driver's license and auto insurance.
• Passion for empowering seniors to live independently.

Responsibilities

• Evaluate and treat patients as ordered by a physician.
• Develop and implement personalized plans of care.
• Educate and support patients and caregivers.
• Collaborate with interdisciplinary team members.
• Supervise PTAs and aides per state regulations.
• Maintain accurate, timely documentation.
• Engage in ongoing education and team meetings.

What We’re Looking For
