Job summary

Kadima Rehabilitation & Nursing at Washington is seeking a PRN Physical Therapist to join our in-house rehab team. The PT will promote residents' physical well-being and functional recovery in a skilled nursing facility.

Responsibilities include evaluating and treating residents, following an established plan of care, instructing patients and families, recording motor function, and documenting per policy and state/federal regulations. PA license in good standing is required.

Qualifications

• SNF/Long term care experience is preferred.
• Current knowledge of treatment practices.
• Knowledge of Net Health Electronic Documentation System is preferred.
• Ability to manage patients with different types of personalities.

• Ensure all treatment is delivered in accordance with an established plan of care.
• Provide clinical support and instruct patients, families, and caregivers.

• Complete documentation in accordance with policy and regulations.

The Physical Therapist (PT) plays a pivotal role in the rehab department at our skilled nursing facility, promoting the physical well-being and functional recovery of our residents.

Duties/Responsibilities:

• Evaluate, plan, and administer treatment to residents and patients with temporary or long-term disabilities to relieve pain, restore or improve function, and promote healing.
• Assured all treatment is delivered in accordance with an established plan of care
• Provide clinical support and instruct patients, families, and caregivers
• Measures & records patient’s motor function, strength, and muscle performance
• Monitors patient responses to treatments and makes modifications during treatment sessions as necessary
• Completes documentation in accordance with company policy and state/federal regulations

Skills:

Required licensure:

• Current License as a Physical Therapist in the State of PA. Must be in good standing.
