Job summary

PharMerica is seeking a PRN Pharmacist for a non-retail, long-term care setting in Lubbock, TX. You will review orders, optimize therapy, maintain patient profiles, and collaborate with physicians and nursing staff to ensure safe medication use.

Role focuses on clinical decision-making, documentation, and compliance with state and federal regulations, with an emphasis on quality care for residents and ongoing professional development within a growing organization.

Qualifications

• Bachelor’s Degree in Pharmacy required; PharmD preferred.
• One year experience in long term care desired.
• Current and valid pharmacist license in the state of practice.
• Knowledge of CMS/DEA/FDA guidelines and medication safety practices.
• Proficient in electronic medical records and pharmacy information systems.

Responsibilities

• Evaluate physician medication orders for drug appropriateness, dosage, interactions, and administration routes.
• Provide clinical recommendations to physicians and healthcare staff regarding therapy changes.
• Maintain patient/resident profiles with medical history, labs, and current medications.
• Ensure laboratory tests for monitoring high-risk meds are ordered, reviewed, and communicated.
• Collaborate with physicians, nurses, and healthcare professionals for clinical support.
• Document communications and interventions in the patient’s medical record.
• Enforce policies for safe handling, dispensing, and medication management; ensure regulatory compliance.
• Report medication-related errors and adverse events to the Pharmacy Manager.
