PharMerica in Grand Prairie, TX is seeking a PRN Pharmacist to join a closed-door, non-retail team serving long-term care and senior living clients. You will evaluate orders, provide clinical recommendations, maintain patient profiles, and ensure regulatory compliance.

The role supports a rotating schedule with weekend coverage and shifts from 8:00 am to 12:30 am in 6–8 hour blocks. Competitive benefits and ongoing training are offered.

Qualifikationen

• Bachelor’s Degree in Pharmacy; PharmD preferred.
• One year of long-term care experience desirable.
• Current and valid pharmacist license in the state of practice.
• Knowledge of CMS, DEA, FDA guidelines and regulations.
• Proficiency with electronic medical records and pharmacy information systems.
• Strong clinical judgment and effective written and verbal communication.

Aufgaben

• Evaluate physician medication orders for drug appropriateness, dosage, interactions, and administration route.
• Provide clinical recommendations to physicians and healthcare staff regarding medication therapy changes.
• Maintain up-to-date patient/resident profiles including medical history, labs, and current medications.
• Ensure laboratory tests for high-risk medications are ordered, reviewed, and communicated to the team.
• Collaborate with physicians, nurses, and other healthcare professionals for clinical support.
• Document all communications and interventions in the patient’s medical record.
• Implement and enforce policies for safe handling, dispensing and oversight of medications.
• Ensure compliance with state and federal regulatory requirements for pharmacy practice.
• Report medication errors and adverse drug events to the Pharmacy Manager.
• Educate and support clinical staff on safe medication handling procedures.
• Perform other pharmacy-related tasks as assigned.
