Job summary

PharMerica in Grand Prairie, TX is hiring a PRN Pharmacist to join our closed-door, non-retail pharmacy team serving long-term care and senior living clients. This role offers flexible 16-24 hours weekly with 5 to 8 hour shifts and rotating weekends.

You will review medication orders, provide clinical recommendations, maintain patient profiles, and collaborate with physicians and healthcare staff to ensure safe, effective therapy and regulatory compliance.

Qualifications

• Bachelor's Degree in Pharmacy required, Pharm D preferred.
• 1 year experience in long term care desired.
• Current pharmacist license in the state of practice.
• Knowledge of drug therapy guidelines, medication safety practices, and regulatory standards (CMS, DEA, FDA).
• Proficiency in electronic medical records and pharmacy information systems.
• Strong clinical judgement and effective written and verbal communication.

Responsibilities

• Evaluate physician medication orders for drug appropriateness, correct dosage, interactions and administration.
• Provide clinical recommendations to physicians and healthcare staff.
• Maintain up-to-date patient/resident profiles, including medical history, labs and current medications.
• Ensure lab tests for monitoring high-risk meds are ordered and reviewed.
• Collaborate with physicians, nurses, and healthcare professionals for clinical support.
• Document communications and interventions in the medical record.
