You will screen, treat, and discharge patients with communication, cognitive or swallowing disorders, following standards and clinical practice guidelines.

Responsibilities

• Screen/evaluate patients with communication, cognitive or swallowing disorders and develop care plans.
• Provide rehabilitative, skilled, and medically necessary treatment interventions.
• Evaluate treatment outcomes and modify care as needed.
• Provide counseling to patients and families regarding speech disorders.
• Prepare comprehensive discharge summaries and maintain records in the EHR system.
