Flagship Therapy's Cedar Hill Post-Acute and Rehab in Jacksonville, FL is seeking a dedicated PRN Occupational Therapist to join our in-house rehab program. You will evaluate patients, develop care plans, and deliver skilled, medically necessary therapy to geriatric clients.

Responsibilities

• Screen and evaluate patients with physical functioning disorders and/or wound care; develop care plans.
• Deliver rehabilitative, skilled, and medically necessary therapy.
• Assist with transfers, standing, walking, and wound care as needed.
• Administer procedures as part of rehab plan (manual techniques, exercises, modalities).
• Provide counseling to patients and families; generate discharge summaries.
