Job summary

Innovate Rehab and Wellness is seeking a Licensed Occupational Therapist to join our full-time team in a Skilled Nursing Facility. You will evaluate patients, develop individualized treatment plans, and help clients regain independence with a focus on safety and functional improvement.

We value a collaborative, patient-centered approach and offer a supportive environment, with Monday–Friday schedules (flexible) and a competitive hourly pay based on experience.

Qualifications

• Master’s or Doctorate in Occupational Therapy (accredited program).
• Valid state OT license.
• Experience as an OT preferred (new grads welcome).
• Strong communication and problem-solving skills.
• CPR certification preferred.

Responsibilities

• Evaluate patients’ functional abilities and therapy needs.
• Create and deliver individualized treatment plans.
• Help patients improve motor skills, coordination, and daily function.
• Support patients in regaining independence and quality of life.
• Conduct thorough OT evaluations.
• Develop and adjust personalized treatment plans.
• Use therapeutic techniques and adaptive equipment.
• Train patients and caregivers on exercises and assistive devices.
• Educate on strategies to improve function and safety.
• Collaborate with PT, SLP, physicians, and the full care team.
• Document patient progress and treatment changes accurately.
• Stay current with OT best practices and research.
