Job summary

Vidor Health in Texas is seeking a compassionate Occupational Therapist (OT) to join our in-house rehab team on a PRN basis. You will focus on improving residents' independence, safety, and quality of life through meaningful daily activities and functional outcomes.

Responsibilities include evaluating patients, developing and implementing treatment plans for ADLs/IADLs, recommending adaptive equipment, documenting thoroughly, and collaborating with an interdisciplinary team.

Qualifications

• Texas license to practice as OT in TX.
• SNF/long-term care experience preferred.
• Knowledge of occupational therapy treatment practices.
• Experience with Net Health Electronic Documentation System preferred.

Responsibilities

• Consult with patients to evaluate their condition and impact on daily occupations.
• Assess and interpret patient evaluations and test results.
• Develop individualized treatment plans to maximize ADLs/IADLs.
• Assess home environments and recommend modifications.
• Recommend adaptive equipment and provide training for independence and safety.
• Deliver treatments per the care plan and monitor responses.
• Collaborate with the interdisciplinary team to ensure high-quality care.
• Complete documentation thoroughly and timely per company/state regulations.
• Attend required Rehab leadership meetings.
