Job summary

Vidor Health & Rehab in Texas is seeking a compassionate Occupational Therapist (OT) to join our in-house team on a PRN basis. You will focus on improving residents' independence, safety, and quality of life through meaningful daily activities and functional outcomes.

This role supports person-centered care in a skilled nursing setting. You will evaluate patients, develop individualized treatment plans, recommend adaptive equipment, and collaborate with the interdisciplinary team.

Qualifications

• Texas OT license in good standing.
• SNF or long-term care experience preferred.
• Familiarity with Net Health Electronic Documentation System.
• Strong communication and clinical decision-making skills.
• Ability to collaborate with an interdisciplinary team.
• Proficient documentation and record-keeping.

Responsibilities

• Consult with patients to evaluate their physical condition and impact on ADLs/IADLs.
• Develop individualized treatment plans to maximize independence.
• Assess home environments and recommend modifications.
• Recommend adaptive equipment and provide training for independence and safety.
• Deliver treatments per the plan of care and adjust as needed.
• Document thoroughly and timely in compliance with policies and regulations.
• Collaborate with interdisciplinary team to ensure high-quality care.
• Attend required meetings and provide education to patients and caregivers.
