Job summary

Stern Therapy Consultants is hiring a PRN Occupational Therapist for Autumn Lake at Glade Valley in Walkersville, MD. The OT will assess daily living skills, upper-extremity function, cognition, and activity tolerance to identify needs and develop measurable goals for discharge planning.

Responsibilities include providing individualized treatment, recommending adaptive equipment, and teaching caregivers techniques for safer, more independent task completion.

Qualifications

• Active Maryland Occupational Therapist license.
• Strong evaluation skills and ability to translate findings into goals.
• Clear communication and accurate clinical documentation.
• Post-acute experience preferred; new grads welcome.

Responsibilities

• Assess daily living skills, upper-extremity function, cognition, and activity tolerance to identify OT needs.
• Develop measurable goals aligned with residents' priorities and discharge plans.
• Provide individualized treatment for self-care, functional transfers, and daily routines.
• Recommend adaptive equipment and teach safety/independence strategies to residents and caregivers.
• Reassess progress, adjust interventions, and supervise COTA services within the plan of care.
• Complete timely clinical documentation and coordinate recommendations with nursing and rehab team.
